STUDENT-ATHLETES:
The Sports Medicine staff will continue to offer the continuity of care regarding injuries sustained at CSUSB. All sports medicine staff are available via email or phone (Office phone: 909-537-5016) within work hours. We will continue our communication with all team physicians via email and phone. PRE-PARTICIPATORY PHYSICAL:
All student-athletes are required to obtain a pre-participatory physical screening. Physicals must be signed by a medical doctor (MD), doctor of orthopedics (DO), or nurse practitioner (NP) in the state of California. Physicals will not be accepted if signed by a physician assistant (PA). You may use our CSUSB Physical Screening Form.
SICKLE-CELL TRAIT TESTING INFORMATION:
Sickle Cell Testing Information:
NCAA Division II Bylaw 17.1.5.1: The NCAA Division II Legislation Committee determined that only results from a sickle cell solubility test satisfy the requirements of NCAA legislation. Results from other types of sickle cell screening do not meet the legislation.

SPORTSWARE:
In addition to a physical screening, all student-athletes must provide the athletic department with current contact information as well as all required forms for Sports Medicine Center. All personal data is secured via our electronic medical record (EMR) called SportsWareOnline. Athletes will receive an email from their coaches with a code and instructions on how to complete their profile before their arrival to CSUSB.

The sports medicine staff wants to welcome you to California State University San Bernardino. Please feel free to contact Pat, Adrian, or Coco for any questions you may have and are not answered on this site.
The Sports Medicine Center (HPE B-02) is located in the basement (next to the elevator) in the Health and Physical Education Building that hosts the Coussoulis Arena.
We have a large area containing seven treatment tables, an over-sized stretching table, and a 15-foot taping table that allows us to service multiple student-athletes. Normatec recovery boots as well as Rapid re-boots, scraping, cupping, electrical stimulation, cardio modalities, and multiple rehab tools are also available within the walls of the Sports Medicine Center.
The following will be available to you and your team on competition day:(excluding Baseball off-campus)
- Sports Medicine Center - open 2 hours before the Athletic Event
- Therapeutic Modalities for treatment as needed (STIM, hot packs, cryotherapy, etc.)
- Certified Athletic Trainer and possibly student interns to assist
- Water cups, water bottles, injury ice
- Emergency bag (Containing AED, Crutches, Splints, etc.)
For special requirements or accommodations, or if you do not plan on travelling with an athletic trainer, please contact us prior to competition.
COACHES:
The Sports Medicine Center opens for business one hour prior to scheduled practice. For any practices prior to 9 a.m., please make prior arrangements with the SMC staff.
The Sports Medicine Center will close 30 minutes after the final practice of the day. For any practices scheduled to end after 5 p.m., please make prior arrangements with the SMC staff.
The Sports Medicine Center will open two hours prior to any scheduled competition. Any needs more than this please make prior arrangements with the SMC staff. Treatments needed after competition need to be done in a timely manner following its conclusion.
SPORTS MEDICINE INTERNSHIP PROGRAM:
The Cal State San Bernardino Sports Medicine Internship program is looking to build a strong and consistent program of providing ethical, high quality, and progressive health care and prevention to its student athletes. CSUSB is providing you with the opportunity to "learn by doing" and observing the skills of an Athletic Trainer, obtain clinical hours for graduate school, and sport assignment rotations at CSUSB. The Sports Medicine Intern (SMI) must demonstrate a level of professionalism that includes self-discipline and motivation to develop knowledge, skills, and experiences.
Students interested in the internship program must:
- Read and complete the Intern Responsibilities & Oath Form.
- Email the completed document, along with a one-page essay stating your interest in our internship, to all athletic trainers.
